Job Overview: We are seeking a dedicated and resourceful RCF Care Coordinator to join our team in Kirksville, Missouri! In this role, you will assist clients in accessing necessary resources in the community, facilitate client admissions, manage census reporting, and work closely with the billing department. The position involves providing orientation regarding program expectations and offering community support services for individuals with mental health and/or substance use disorders. Key Responsibilities:

Perform the duties of a Community Support Specialist, including documenting services and interventions related to client treatment plansMeet with existing clients to encourage utilization of offered services and provide case management as necessaryDevelop and maintain community partnerships to help clients obtain medical care, insurance, transportation, and other essential servicesAssist with the orientation of clients on their rights and program rulesDevelop discharge and aftercare plans to ensure successful transitions to continued servicesEnsure effective communication and coordination with other service providers, medical professionals, and referral sourcesComplete all mandatory training within regulatory standards, maintaining a waiting list and managing program census as applicableRespond to new referrals and maintain positive relationships with clients, coworkers, and referral sourcesFacilitate screenings and coordinate admissions in electronic medical records and applicable databasesProvide exceptional customer service to create a welcoming environment for new clients/patientsAssist in filing, quantitative chart review, and compiling chart information as neededRepresent the agency professionally and adhere to the code of ethics and agency policies

Requirements:

Bachelor’s degree in a human services field or a combination of higher education and qualifying experience in mental health or substance use servicesValid driver’s license with an acceptable driving record and current auto insuranceSuccessful completion of a background check including criminal record, driving record, abuse/neglect, and fingerprint checksCompletion of New Hire Orientation and all required training within set timelinesAbility to communicate effectively both verbally and in writing, with strong organizational and time management skillsKnowledge of confidentiality laws and case management principles related to mental health and substance use disordersMust be at least 21 years of age to operate company vehicles and at least 23 years of age to operate a 15-passenger van
