Job Summary:

In this role, you will be responsible for reviewing and responding to documentation correction requests, serving as a liaison between providers, departments, and technical teams. Your expertise will help mitigate compliance and financial risks while ensuring that all medical records align with organizational policies and regulatory requirements.

Key Responsibilities: 
• Review and process provider requests for corrections to medical record documentation in a timely and accurate manner. 
• Evaluate correction requests and recommend alternative solutions that maintain the integrity of medical records. 
• Analyze potential risks associated with documentation inaccuracies, including privacy concerns and financial overpayments. 
• Coordinate communication between providers and internal departments to ensure complete and accurate resolution of chart corrections. 
• Maintain organized and detailed documentation of analyses, findings, and recommendations. 
• Collaborate with IT and revenue cycle teams to ensure compliance with policies, procedures, and regulatory standards. 
• Ensure adherence to all applicable federal and state documentation regulations. 
• Utilize spreadsheets and reporting tools to track and monitor correction activities. 
• Provide education and guidance to staff on documentation standards and correction processes. 
• Perform all other duties as assigned. 

 

 

Education, Experience, and/or Credential Qualifications: 
• High school diploma or equivalent. 
• Associate’s degree in health information management preferred. 
• RHIA or RHIT credential strongly preferred. 
• Two years of medical records experience required. 

 

 

Additional Qualifications: 
• Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check. 
• Completion of New Hire Orientation at the beginning of employment. 
• All training requirements including Relias at the beginning of employment and annually thereafter. 
• Current driver’s license, acceptable driving record, and current auto insurance. 

 

 

Physical Requirements: 
• ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift. 
• Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
