Recovery Coach (Healthcare for the Homeless)

Cornell Scott-Hill Health is seeking a Full-Time Recovery Coach.
The Recovery Coach works collaboratively with staff to provide motivational engagement, inspirational support, connections to resources and linkages to clients as indicated by the Recovery Coach’s supervisor. The Recovery Coach has demonstrated motivation to use his/her experiences to help inspire those still struggling through sharing their stories and providing supportive activities.
Responsibilities include but are not limited to:
+ Advocates for patients to engage in appointments and meetings as required to ensure seamless connection and to avoid premature drop-out.
+ Works to improve client retention at the onset of engagement.
+ Maintains patient confidentiality at all times and follows agency, state, and HIPAA laws.
+ Establishes and maintains good working relationships with community partners.
+ Attends and participates in team conferences, as required.
+ Ensures required documentation is complete, timely, and accurate in Epic, HMIS/DMHAS.
+ Engages and documents on a minimum of 30 clients monthly in HMIS.
+ Performs all other duties as required.
Requirements:
+ Completion of Peer or Recovery Coach training program (or equivalent).
+ High School Diploma / G.E.D OR must be in pursuit of through Adult Education or other avenues.
+ Bilingual (English/Spanish) preferred.
+ Self-identified person in recovery who has maintained sobriety for a minimum of two years.
+ Extensive knowledge of recovery-based supports, both traditional and non-traditional, and ability to integrate these resources into treatment.
+ Unbiased, working knowledge of methadone and other MATs as tools to support recovery. K nowledge working with LGBTQ+ community and resources.
+ Excellent communication skills, both written and verbal.
+ Ability to work with an interdisciplinary team.
+ Knowledge and skills related to individual, group, and family system dynamics.
+ Must have a valid Connecticut driver’s license and daily access to a car in order to attend meetings/ outreach events.
+ The employee must occasionally lift and/or move up to 40 pound backpack over uneven ground throughout the city.
+ While performing the duties of this job, the employee must be able to work in a fast-paced environment with minimal to high volumes of stress.
+ The employee must be able to conduct street outreach for long periods during the workday and in all weather conditions (snow, rain, and during severe weather conditions).
