Due to our continued commitment in supporting people suffering with substance misuse problems, Change Grow Live are looking for a Recovery Coordinator - Specialist In-Hospital Liaison  to join our busy Birmingham service working in Heartlands hospital.

 

Our ideal candidate will have the skills and experience to offer brief interventions to patients in a hospital setting, supporting them to access drug and alcohol treatment This is an excellent opportunity for someone already working within a similar field looking to take the next step in their career with a charity that will support your continued personal and professional development.

 

Where: Birmingham  

Full Time Hours: 37.5hrs per week

Full Time Salary Range : £27,449.53 - £31,529.41 (Pro rata for part time hours)

Responsibilities

About the role:

Set up effective communication and referral pathways with hospital-based specialist teams, acting as a point of contact for referrals.Work in partnership with hospital wards to identify new clients and repeat attendees to enable them to engage in treatment. Develop specific links with hospital wards including A&E, AMU, Cardiology, Respiratory and Psych Liaison and other specialist services. To support the engagement of patients.Provide case management to small caseload of patients who misuse alcohol and drugs to support their engagement in community treatment services. Work with community teams to ensure assertive and effective follow up of both repeat presentations and discharges from the hospital.Provide support, advice and training to ward staff of those identified as misusing alcohol and/or drugs to prevent further complications from their conditions both in the short term and the long term.Maximise the opportunity to support clients in hospital including triage for their substance misuse, providing harm minimisation and brief psychosocial intervention.

About you:

Have a strong understanding of substance misuse issues and experience of working within a similar field Be an excellent communicator, both verbal and written with good IT skillsProactive and able to work on own initiative with excellent time management and prioritising skillsHave a good working knowledge of mental health interventions, services, and good practiceBe a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholdersAbility to manage change successfully in a way that prioritises the needs of service usersUnderstand the importance of information governance processes and commit to follow and apply all necessary safeguards
