To maintain supportive contact with residents to continually assess their individual needs, especially during initial period of residency and during stressful and crisis situations.
To provide advocacy/case management services whenever appropriate for each resident.
To develop strong, supportive relationships with the clients.
To develop, cultivate and manage a positive culture and milieu.
To provide for client care and safety and crisis intervention when necessary.
To notify Program Director immediately in the event of a client crisis, suspected illegal activity or any behavior constituting an incident in the program and handle these situations in the Director’s absence.
To interact with clients in order to assess needs, provide information, discuss problems, set limits and encourage growth and development and to monitor each client’s progress.
To provide residents with an understanding of house routines, rules, and responsibilities (including curfew routines, sleeping quarters, maintenance and household chores, etc.).
To develop and maintain a working knowledge of local community resources and social service agencies and develop positive and sustaining working relationships with them.
To maintain updated clinical notes and reporting in accordance to established record keeping procedures.
To participate in all case consultation meetings, staff meetings and case conferences, as appropriate.
To provide orientation of services to prospective clients, family, and community groups.
To facilitate the Family Orientation Group, the Consumer Advisory Committee, and the Alumni Support Group, and maintain minutes thereof.
To receive, review and address all consumer complaints and grievances.
To provide recommendations for discharge planning and aftercare services.
