Baystate Health, a nationally recognized leader in healthcare quality and safety, and home to Baystate Franklin Medical Center is looking for a grant funded Recovery Support Navigator to join our Substance Use Disorder Team team in Greenfield, MA!

*Option of fully remote available unless needed on site for meetings.

 * This position also supports Substance Use patients in Westfield, Palmer, and Springfield areas.

RESPONSIBILITIES:
Baystate Health, Recovery Support Navigator will be key to supporting the provider efforts to ensure services requested are provided and key contacts/follow ups occur.  This position is grant funded from the Bureau of Substance Abuse Services to support additional addiction services in the three community hospitals. The Recovery Support Navigator is responsible to connecting Substance Use patients with resources for follow up treatment and services. The incumbent is also responsible for building relationships with organizations who provide these treatments and services to optimize exceptional patient care.

SCHEDULE:

40 hours per week- 8:00am-4:30pm No weekends, No holidays

LOCATION:

Baystate Franklin Medical Center Greenfield, MA Fully remote option within a commutable distance to be on-site for occasional meetings

QUALIFICATIONS: 

High School Diploma/GED Understanding of Harm Reduction/Pathways to recovery Knowledge of community resources as it relates to substance use and public assistance programs Bilingual, preferred
