We are looking for an energetic individual to join our Health Home Care Management Team! The ideal candidate will be someone who exercises compassion and dedication to serving the vulnerable members of our community. Our philosophy is that recovery happens through therapeutic relationships. The sooner we can connect individuals who need ongoing Care Coordination, the sooner the member can begin to reach his or her potential. This includes the promotion of preventative care to reduce preventable emergency department and inpatient utilization, as well as an opportunity to address any social determinants of health.

Position Summary:

Responsible for managing and processing Heath Home referrals for the region. Maintaining up-to-date knowledge on Health Home eligibility and appropriateness criteria. Communicating with referral sources to provide status updates, as well as contracted Care Management Agencies (CMA’s)regarding capacity needs along with assigning referrals to meet those needs.

This Referral Specialist also manages the Health Home main phone line and email inbox, often serving as the initial point of contact for the community and stakeholders. This includes, but is not limited to, to answering questions regarding The Health Home Program, how to place referrals, and provide status updates on current referrals.

In addition, the Referral Specialist will support health home leadership and staff in development of health home marketing and outreach materials and strategies. Develop and demonstrate knowledge of the NYS Health Home goals and criteria along with care management model.  

Education & Experience
Requirements:

Associates degree in Human Services with minimum of two years’ experience. Previous discharge planning, counseling, case management, and substance abuse treatment experience is desired.Strong writing and communication skills are required, as well as knowledge of working with community agencies and managed care representatives. Experience working with a diverse population and a strong understanding of multicultural issues is preferred.A valid and insurable NYS Driver’s License.

Responsibilities:

Responsible for managing and processing Heath Home referrals for the network.Enter and maintain accurate, comprehensive data entry of all referrals into the database.Verify all pertinent documentation has been provided for the type of referral received. Document all stages of the referral in the appropriate data base or in writing. Initiate engagement activities via phone call and mailing letter(s).Complete all documentation within required timeframes (as defined in CHC Policies). It is the expectation that all interactions with or on behalf of a Health Home Member be documented in the electronic health record and be unique and detailed.Be an engaged team member who supports colleagues and department needs. This includes participation in team and department meetings, as well as supervision.Readily assist other staff members upon completion of assigned duties. 

Pay Range: $20.00 - $26.67

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
