Registered Dietitian (RD) Diabetes Educator
POSITION SUMMARY: The Diabetes Dietitian Educator position for the Diabetes Self-Management Program is a Registered Dietitian who provides effective diabetes education programs for patients and their families in accordance with the National Standards for Diabetes Self-Management Education, functioning as an integral member of a multidisciplinary team. EDUCATION/CERTIFICATION: + The Diabetes Dietitian Educator must be registered as a Registered Dietician in the State of CT, with a bachelor's degree in a related field. + Certification by the National Certification Board for Diabetes Educators as a Certified Diabetes Educator (CDE) is required. EXPERIENCE: + Two years recent didactic and experiential preparation in education and diabetes management is strongly preferred. COMPETENCIES: + Excellent presentation and interpersonal skills are required. + This position requires a working knowledge of Microsoft Office software, basic familiarity with WORD and OUTLOOK. Job-Specific Competency 1. Provides effective individual and group Diabetes Self-Management Education and Medical Nutrition Therapy, individualizing teaching plans. Chooses appropriate educational materials and interventions based on assessment of each patient's needs and the National Standards of the American Diabetes Association. 2. Promotes positive behavioral changes to control risk factors for complications, via goal-setting. Schedules additional visits/telephone calls as appropriate to assist patients to reach or revise goals. 3. Documents each patient's assessment, education plan, intervention, evaluation, and follow-up in the permanent confidential education record visits per department procedure, within 48 hours of patient visit. Documentation will show evidence of collaboration among instructional staff, providers, and referral sources. 4. Responds to telephone calls from patients within 24 hours. Schedules follow-up appointments in Meditech. Ensures that every patient seen has at least one follow-up visit, phone call or other contact. 5. Calls or arranges calls to patients who did not attend a scheduled visit or class to re-schedule. 6. Participates annually as part of the Advisory Board in a planning and review process that includes data analysis and outcome measurements, and addresses community concerns. Participates in ADA program re-certification process. Works with team members day-to-day to evaluate the effectiveness of the education experience provided, and determine opportunities for improvement. 7. Obtains regular continuing education in the areas of diabetes management, behavioral interventions, teaching and learning skills, and counseling skills. 8. Participates in departmental meetings and programs, special projects and community activities. Actively supports DSMP development and expansion. 9. Orients new staff and precepts dietetic students when requested.
