FLSA Status: Exempt Original Date: April 2004 Last Revision: December 2020 JOB SUMMARY: Provides coordination of care services for patients receiving dialysis therapy throughout the medical center and enhances communication with outpatient dialysis centers. Coordinates quality improvement initiatives to facilitate compliance with hospital and regulatory standards of care. EDUCATION/EXPERIENCE: Graduation from an accredited School of Nursing with three years? dialysis experience; or, an equivalent combination of education and experience. Inpatient dialysis experience preferred. LICENSURE, CERTIFICATION, and/or REGISTRATION: Registered Nurse licensure from the State of North Carolina Basic Cardiac Life Support (BCLS) required Advanced Cardiac Life Support (ACLS) required within 1 year of hire for Adult Critical Care, Adult ED Pediatric Advanced Life Support (PALS) required within 1 year of hire for Network ED, Peds ED, PICU and NICU, and other areas per unit guidelines ESSENTIAL FUNCTIONS: 1. Exemplifies the responsibilities and accountabilities inherent in a professional nursing practice. 2. Collaborates with the health care team as well as with the patient and family to develop goals and evaluate care outcomes through consultation and information sharing. 3. Utilizes the nursing process to ensure quality patient care is provided throughout the episode of care. 4. Creates and maintains a safe and therapeutic environment for patients, self, co-workers, and facility. 5. Ensures complete, accurate, and timely written communication of patient information. 6. Provides a therapeutic environment for safe, accurate, and timely medication and IV administration. 7. Assumes responsibility for the nursing components of the patient/family/significant other educational process. 8. Communicates to the team, co-workers, patients, and family members in an effective, positive, and professional manner. 9. Takes personal responsibility and initiative for performance and for professional growth and development. 10. Provides coordination of care and provision of services for patients receiving dialysis therapy throughout the medical center, and enhances communications with outpatient dialysis centers.
