The PFK Care Coordination Team Lead meets the responsibilities and competencies of the Care Coordinator RN role. The PFK Care Coordination Team lead works closely with the Program Manager to ensure all aspects of the care coordination program including care transitions, case management and ED diversion programs meet expectations. This position requires a Registered Nurse who is able to direct the work of other Registered Nurses, Social Workers, and non-clinical staff. The PFK team lead assists in supervising department operations, planning, evaluating and controlling program services to ensure that internal and external standards are met.

Responsibilities

1. Actively provides case management to high risk complex cases.

2. Assists with maintaining departmental staffing needs in accordance with budget and established ratios. a. Monitors time and attendance b. Schedules staff

3. Provides training, ongoing education and leadership to clinical and non-clinical staff to ensure consistency in performance

a. Orients new staff

b. Designs and implements Ongoing education staff programs

c. Provides performance feedback, corrective action and evaluation

d. Serves as a Role Model, Preceptor, and Mentor

e. Assess Clinical Competency

f. Provides reward and recognition

g. Assists with clinical decision making, problem solving and resource allocation

h. Ensures staff participation in department and hospital initiatives

i. Demonstrates use of the nursing process and evidenced base practice

4. Assist in activities to evaluate program processes and implement process/performance improvement initiatives.

a. Analyzes data to justify and correct variances.

b. Generates reports to monitor effectiveness and compliance

c. Conducts audits of documentation and practices

d. Develops guidelines and protocols

5. Provides leadership to department staff

a. Enlists staff participation in department and hospital initiatives.

b. Oversees staff performance

c. Assists with clinical decision making

6. Provides assistance to and coverage for the Program Manager

7. Supports department activities to ensure that they meet regulatory standards related to

a. NCQA standards

b. NCH safety and quality programs

c. Delegated Responsibilities

d. Ohio Department of Health

e. Joint Commission

f. Department and hospital policy and procedures

8. Assist with preparation and submission of oversight documentation and reporting to entities contracting with PFK Care Coordination department.

9. Assists with coordinating research, patient safety, quality improvement activities and special projects. 10. Practices professional nursing in accordance with the ANA code of Ethics for Nurses.

11. Collaborates with leadership team with PFK and NCH in program development.

12. Communicates clearly with internal and external customers

13. Demonstrates accountability for professional growth by reading current literature, participating in professional organizations, and attending hospital and community education.

Qualifications

KNOWLEDGE , SKILLS AND ABILITIES REQUIRED

1. BSN required

2. Licensure: Licensed as a Registered Nurse in the state of Ohio

3. Certification preferred

4. Three years’ experience in pediatric clinical care or case management

5. Travel and home visiting within a 34 county area expected.

6. Good Driving record is required

7. Proof of auto insurance

8. Active driver’s license
