This is a direct care opportunity to help your clients, who may struggle with accessing standard care, meet their goals and ensure they receive the treatment they need.
Specific responsibilities include:
Helping clients achieve their treatment goals in partnership with the treatment team. Communicating and coordinating services with outside medical providers. Overseeing medication management under the direction of the team prescriber. Developing and maintaining professional relationships with clients, their support system, community agencies, and the Sheppard Pratt organization to provide comprehensive direct care.What you’ll get from us.

An associate or bachelor’s degree in nursing.
