Key Responsibilities:
Diabetes Self-Management Education: Conduct individual and group education sessions for patients and their families on managing diabetes. Develop personalized care plans to promote effective diabetes management, including lifestyle modifications and medication adherence. Monitor patient progress and adjust education plans as necessary to achieve optimal glycemic control. Gender Dysphoria Care: Administer hormone injections and provide ongoing support for patients undergoing gender transition. Educate patients on the effects and management of hormone therapy, ensuring informed consent and understanding. Collaborate with multidisciplinary teams to provide holistic care for transgender patients. Prior Authorizations and Prescription Refills: Manage and process prior authorizations for medications and treatments, ensuring timely approval and delivery. Coordinate with pharmacies and insurance companies to resolve any issues related to medication access. Handle prescription refill requests efficiently, maintaining accurate records and communication with prescribers. Triage and Patient Communication: Triage patient calls, assessing the urgency of symptoms related to multiple endocrine disorders, including thyroid, pituitary, and adrenal conditions. Provide clear and compassionate communication, offering guidance and advice as needed. Coordinate follow-up care and appointments based on triage assessments. Collaboration and Professional Development: Work collaboratively with endocrinologists, dietitians, social workers, and other healthcare professionals to deliver comprehensive care. Stay current with the latest developments in endocrine nursing and participate in continuing education opportunities. Contribute to the development and improvement of clinic protocols and patient education materials.
