Under the guidance and direction of the Director of Patient Services and Home Care Manager, the After Hours RN will provide intermittent assessments, direct physical nursing care, and other home visits needed per the PACE CNY Clinic and the Answering Service/Triage RN Service. The After Hours RN is responsible for providing the highest quality of care in their home care and ensuring safety at all times according to accepted nursing practice standards. The RN acts as a primary support to the Home Care Manager.

The practice of the profession of nursing as a Registered Professional Nurse is defined as diagnosing and treating human responses to actual or potential health problems though such services as case finding, health teaching, health counseling and provision of care supportive to or restorative of life and well-being, and executing medical regimens prescribed by a licensed or otherwise legally authorized Physician or Dentist. A nursing regimen shall be consistent with and shall not vary any existing medical regimen. (Education Law, Article 139)

 

Responsibilities

Job Duties Specific to This Position:

Under the supervision of the Director of Patient Services and/or the Home Care Manager:

Makes home visits to assess, organize, implement and evaluate participant care and reports changes and concerns to the ON-call Home Care Manager, Triage RN Service, or Provider, as appropriate.Demonstrates appropriate knowledge and skill while performing quality care and meeting the participant specific needs, as evidenced by appropriate documentation.Provides appropriate education to participant and family and appropriately documents including the use of educational handouts and effectiveness of education.Completes aide education, supervisions and orientations in a thorough manner as directed.Obtains, transcribes and implements physician orders per request accurately.Documents all nursing visits, phone calls, orders, etc. in a thorough manner within 48 hours of visitFollows policies related to medication administration, transcription and MAR process.

 

Provides Leadership among co-workers.

Demonstrates effective communication skills while working cooperatively with Home Care staff, PACE staff, and outside providers, including the answering service/triage RN service.Promotes team building be demonstrating an active role in staff meetings, orientation of staff and welcoming new staff.Actively participates and demonstrates understanding of QAPI process.Promotes professional growth, the profession of nursing and the program, when possible, by participation in profession associations, educational conferences, volunteer activities, etc.Demonstrates adherence to all compliance policies and procedures and the code of conduct.Adheres to all compliance policies and procedures and regulations. Promotes, fosters and displays compliance oriented behavior in the workplace.Performs responsibilities according to the highest quality standards.Practices safety awareness, reports any possible safety/environment issues to supervisor and adheres to safety P&Ps

INDNS

Qualifications Degree from an accredited school of nursing for RN’s.Current NYS license and registration as a Registered Professional Nurse required.Minimum of one year recent experience as an RN in acute care, long term care, or physician office required.Prior Home Care experience preferred.Must have a valid driver’s license without restrictions and a reliable, insured vehicle.Proven patient assessment skills.Current knowledge of best practice nursing standards and careExcellent interpersonal skills and the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.Ability to be involved with family member or other support persons while providing care.Excellent organizational skills.Ability to read, write, and speak clearly in EnglishProficient in MS Office and electronic medical record programs.Must be in good general health and free of addictions to drugs, alcohol and other substances that might compromise the rendering of safe care to the participant.

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
