Hourly Rate: $39.07 - $56.00

Hours of work: Monday - Friday, Days.
Location: Humber River Health - Wilson Site
Responsibilities:
Assesses, plans, implements and evaluates patient care in alignment with the RN scope of practice outlined by the College of Nurses of Ontario.Maintains the health record according to CNO documentation standards and Humber River Health Policies and ProceduresIntegrates best practice guidelines and evidence based research into clinical practice related to the geriatric and adult populationsKeeps abreast of new developments in Geriatric Nursing and related evidence-based advancements with the intention of integrating into clinical practiceFacilitates movement through transitions of care, ensuring patients are connected with the appropriate services, resources and supports within HRH and in the communityProvides health education for patients and families regarding chronic disease management, diagnostic tests and prescribed treatmentsLiaises with the patient’s primary care physician or primary care provider and community agencies as required throughout the care processEnsures that the patient or caregiver is included in development of the care plan and understands the medical advice or recommendations.Demonstrates the ability to manage intravenous infusions (i.e. blood, antibiotics), blood collections, and support other medical day procedures as needed.Able to provide timely patient support by triaging phone and email communicationsEffectively navigate internal and external referral partners to support patient careCollaborates with health care providers in other units/areas in regards to patient referrals or issues Demonstrates the ability to navigate and perform computer competenciesWorks collaboratively with the interdisciplinary team to support the ongoing functioning of the Outpatient Medical Ambulatory Clinics related to referrals, consultations and follow-upsAs assigned by Manager or delegate, triages clinic referrals according to the clinic specific process and timelineRequirements:
Current Ontario College of Nurses Registration in good standingCurrent BCLS certification requiredExperience with the assessment of various geriatric syndromes and adult populations Demonstrates knowledge of geriatric nursing management, cognitive tests, delirium, dementia and depression (3Ds)CNA certification in Geriatrics would be an assetAbility to adapt and effectively work in a fast-paced environment with changing priorities and time sensitive targetsDemonstrated good patient assessment skillsHigh caliber of observational skills and an ability to react quickly and appropriately to crises situationsEvidence of sound judgment and initiativeMust possess excellent organizational skillsPhysically able to stand, walk and sit for prolonged periods of timeDemonstrates excellent patient and family teaching skills that encompass adult learning principlesDemonstrates effective problem solving skills and an ability to work well independently with minimal supervision and within teamsHighly effective interpersonal and communication skills are requiredSensitivity to and awareness of cultural, racial, economic and socially diverse communitiesExcellent attendance and discipline free record required.Why choose Humber River Health?
