ESSENTIAL DUTIES AND RESPONSIBILITIES The Registered Nurse will conduct comprehensive initial and recertification assessments and supervisory visits for a Licensed Home Care Service Agency (LHCSA) and develop a plan of care in accordance with the physician’s orders and with the policies and procedures of the agency, the state’s professional standards and regulatory entities. Additional duties and responsibilities include: · Assess clients in their home at start of care to develop a plan of care · Initiates Plan-of-Care and makes necessary changes as patient's needs evolve · Conduct minimal six home visit/assessments on a bi-weekly basis. · Reassess assigned clients every six months to determine any condition change as well as supervise the home health aide to ensure proper care · Perform change in condition visits · Supervise/assess Home Health Aides in the client's home every four months, as assigned · Educate clients about health maintenance and disease prevention COMPETENCIES · Excellent assessment skill · Excellent organizational & time-management skills · Basic computer skills; knowledge of data base entry and email (i.e. Outlook) · Skilled in interpersonal relationships · Excellent documentation skills, ability to meet deadlines · Ability to carry out instructions and maintain records · Ability to assume delegated authority and accountability for care of assigned patients · Ability to function independently with minimal supervision EDUCATION EXPERIENCE NYS licensed and currently registered as a Registered Professional Nurse Qualifications It is the policy of RiseBoro to provide equal employment opportunity to all employees and applicants for employment.
