Schedule: 40 hours/week; 1st shift, Monday-Friday
Earn $36.68/hour plus mileage reimbursement
Position Details
The full-time registered nurse (RN) plays a pivotal role in our community-based Dual Diagnosis Treatment Team (DDTT), serving the Northeast Pennsylvania region. The dual diagnosis team takes a compassionate, recovery-oriented approach dedicated to providing high-quality care and unwavering support for individuals diagnosed with both a mental illness and an intellectual disability. The RN will spend their time in the community, directly interacting with consumers in their homes, traveling, and documenting their work.
Duties and Responsibilities
Collaborate with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan. Perform physical health medical monitoring, assessment, etc. Support implementation of intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences. Complete accurate documentation in the electronic medical record. Participate in shared on-call rotation.Benefits
