Summary:
This is a part-time position. The Registered Psychiatric Nurse (RPN) provides screening assessment and psychiatric nursing services on the Healthy Transitions Team (HT).This multi-disciplinary team is designed to provide outreach comprehensive evidence-based treatment referral and support services for a population of youth and young adults and their family members struggling with severe mental illness and co-occurring substance use. The goals of the Healthy Transitions Team Nurse are:1) To assist clients with meeting personal objectives for improving and maintaining mental health especially regarding medication management and compliance; 2) Identify and provide intervention for youth and their families who are experiencing or at risk of developing substance use disorder; and 3) Promote improve and sustain the overall wellness and physical health of the target population.
Responsibilities:
MAIN DUTIES AND RESPONSIBILITIES:
Maintain knowledge of the Healthy Transition�s Coordinated Specialty Care evidence-based treatment model.Meets with Healthy Transitions Team staff members contributing knowledge and expertise to the team in case review and team planningProvides psychiatric nursing interventions referral and care coordination services for enrolled youth and their familiesProvide relevant education to client and familiesSupport clients as they work towards their goalsAttend agency and Healthy Transitions staff meetings as scheduled.Attend weekly supervision as scheduled.Maintain detailed accurate and timely documentation in medical record.Enhance and maintain professional expertise through educational activities and trainings.Attend coordinated care meetings and report issues to Healthy Transitions Team LeaderConducts screenings Biopsychosocial assessments high-level care coordination and tracking of enrolled youth and their families to ensure follow up and connection with referrals madeEngages in trainings on Coordinated Specialty Care model evidence-based wrap around models motivational interviewing cognitive behavioral therapy and practices and supported employment implementing learned skills in daily interventionsParticipates actively in care coordination with community and health providers and engages in multi-disciplinary review meetings to develop integrated care plans for enrolled youth and familiesEngages in cross training of ICT staff and FQHC Clinician on Serious Emotional Disturbance Serious mental illness substance use disorders chronic disease and health conditionsCompletes screenings utilizing standardized tools for Substance use and mental health including SBIRT trauma screening and tools for depression anxiety suicidal ideation and social determinants of healthEngages in screening for general physical health including body mass index sleep issues asthma oral health immunizations wellness visits tobacco use and exposure communicable diseases and other infectious diseases; facilitates referrals as appropriate and conducts follow up to ensure connection and follow throughEngages and maintains collaborative positive relationships with a network of specialized providers to address needs of population including nutrition education physical activities smoking cessation health literacy housing social and rehabilitative programs and job training and placementAssists team in timely completion of GPRA outcome toolMaintains standards for quality care productivity and completion of clinical documentationAssists program manager in the completion of monthly progress reports and health reports to Department of Behavioral Health Developmental Disabilities and Hospitals; track and submit identified metrics�Follows agency policies and standards for client rights and confidentiality

Other information:
Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
QUALIFICATION REQUIREMENTS:
�Associate or bachelor�s degree in Nursing
�Minimum of two years experience working as a psychiatric nursing in a community based behavioral health setting
�Valid and current Rhode Island Registered Nurse License
NECESSARY SPECIAL REQUIREMENTS:
�Valid drivers license and use of car during normal working hours or other means of transportation to complete major functions

�Automobile liability insurance coverage for vehicle being used in accordance with Center requirements.
�CPR training within 90 working days of employment and maintained annually.
