Join our dedicated team of nurses at HeartShare Human Services of New York, where you will play a vital role in enhancing the health and quality of life for the individuals we serve. We are currently seeking Registered Nurses (RNs) for full-time, part-time, and on-call positions.

 

As an RN, you will:

 

Provide specialized nursing care and support to individuals with IDD, promoting their independence and addressing their unique healthcare needs. Advocate for the healthcare needs and rights of individuals, ensuring they receive the best possible care. Train Direct Support Professionals (DSPs) and Approved Medication Administration Personnel (AMAP) in nursing tasks and supervise their performance. Oversee health services for individuals in residential settings and ensure coordination with medical and clinical teams. Maintain thorough documentation, including nursing assessments, post-ER visit follow-ups, and medication management. Collaborate with medical specialists and clinical staff to deliver comprehensive care. Conduct medication inventory, certify AMAP staff, and ensure compliance with OPWDD health training requirements. Participate in program planning and attend nursing department meetings. Respond to emergencies promptly and provide individualized nursing interventions.
