In this role, you will be responsible for performing nursing assessments, monitoring health status, and collaborating with treatment teams to develop action plans tailored to each client's needs. Your clinical competence will ensure that clients receive the highest quality of care in a variety of settings.

Key Responsibilities:
· Perform nursing assessments and evaluate client/patient physical conditions to determine service needs.
· Create and implement action plans based on clients' physical and mental health treatment needs.
· Monitor health status and coordinate care with the treatment team to ensure comprehensive support.
· Administer medications and oversee compliance with state regulatory standards.
· Provide health education and disease prevention strategies to clients and their families.
· Arrange medical appointments and manage emergencies, advocating for necessary medical services.
· Maintain accurate and timely documentation in accordance with company policies.
Education, Experience, and/or Credential Qualifications:
· Current Registered Nurse licensed in the state of practice.
· Must be willing to become licensed in additional states.
· Must be comfortable providing telehealth services.
· BCLS certification required; ACLS certification preferred.
Additional Qualifications:
· Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check.
· Current driver’s license, acceptable driving record, and current auto insurance.
