Registered Nurse Cardiac Rehab- PRN, Days

Job Summary:
The Cardiac Rehab Nurse is under the direction and supervision of a physician. Assignment of responsibility in patient care (direct or indirect) is by the Nursing Director of the Department. He/she is responsible for the coordination of direct and indirect care within the parameter of the Kentucky Nurse Practice Act and the policies and procedures of Baptist Hospital Northeast. Responsibilities are commensurate with the education preparation and demonstrated competencies. The RN is responsible to assist in the fulfillment of the hospital mission.
Responsibilities:
Obtains accurate nursing history from patient, family, or significant others to complete appropriate admission forms.
Identifies cardiovascular risk factors, manifestations of disease and lifestyle modification needs.
Reviews diagnostic data such as serial EKG’s, serial enzymes and exercise test results.
Assesses needs of patients by participating in Cardiac Rehab staff meetings, patient conferences and PI conferences and collaborating with physicians and other Cardiac Rehabilitation team members.
Individualizes Cardiac Rehabilitation protocol or care plan.
Involves patients and family member in program discharge planning.
Implements physicians’ orders for Cardiac Rehabilitation.
Coordinates care with other Cardiac Rehabilitation members and directs technical staff accordingly.
Documents information in the electronic health record.
Collaborate with Cardiac Rehabilitation team members to evaluate response of patients to plans of care. This includes evaluation of age-related needs.
Evaluates goals of Cardiac Rehabilitation Program.
Teaches self-care concepts for lifetime maintenance of risk factor reduction of health cardiovascular habits.
Requirements:
Active Kentucky License.
Basic Cardiac Life Support (BCLS) required.
Advanced Cardiovascular Life Support (ACLS) required.
6 months of previous experience preferred.
