Aspirus at Home in Iron River, MI is seeking a Registered Nurse- Case Manager to join our team!
The Registered Nurse, Case Manager is a professional who assumes the responsibility and accountability for the provision of assessment, planning, implementation, coordination, monitoring and evaluation of nursing care options and services provided to Home Health & Hospice patients of all ages and their families to meet the patients’ health needs through communication and available resources across the health care continuum in all settings. The Registered Nurse, Case Manager participates in the development of care plans in partnership with Home Health & Hospice employees and volunteers, Aspirus, Inc. entities and community partners.
HOURS: This position is Full time; 1.0 FTE or 80 hours per two week pay period. On call rotation would require night work if called in. Day hours only when not on call.
Experience/Qualifications
Knowledge of patient care standards normally acquired through completion of a Diploma, Associate Degree or Bachelor’s Degree in Nursing, an active Wisconsin and or Michigan nursing license and current CPR certification.Previous experience working in home health or hospice and five years of broad-based nursing experience with additional training or experience with the terminally ill beneficial.Possesses an active Wisconsin/and or Michigan RN nursing license.Current BLS certification or obtained within 90 days of hire - All clinical personnel in identified job codes are required to maintain Basic Life Support (BLS) training through independent study, manikin and knowledge testing. An acceptable CPR training program is the Basic Life Support (BLS) Health Care Provider course from the American Heart Association or the American Red Cross Professional Rescuer Program with AED. (See Cardiopulmonary Resuscitation Training Policy).Possesses a valid Wisconsin or Michigan Driver’s License required.Is able to travel to patient/client homes to provide patient/client care.Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required.Respects maintenance of patient/family rights, provision of excellent home health and hospice care, promotion of patient/family involvement in care, respect for spiritual and cultural beliefs, patient advocacy, and promotion of quality of life.Behaves in a professional manner in all contacts related to home health and hospice employment, respects professional boundaries in relation with patients and families, maintains confidentiality, and acts as a positive ambassador for Aspirus.Participates as a positive, contributing member of the home health and hospice team through behaviors which demonstrate cooperation, collaboration, effective communication, initiative, dependability, accountability for one’s actions, willingness to learn and teach, and team support.Maintains corporate compliance by following all applicable federal, state, corporate, and other regulations and policies.Demonstrates respect for governmental, public, private, and corporate funding of home health and hospice care at all times through cost-effective practices.Demonstrates behaviors reflective of Aspirus’s mission, vision, goals, and ethics at all times.Employee Benefits
Aspirus Iron River Hospital & Clinicsis one of four hospitals in the Upper Peninsula of Michigan in the Aspirus family.
With 300+ employees, Aspirus Iron River Hospital and Clinics serves people through a 25-bed hospital, physician clinics, dialysis center, rehab fitness center, ambulance service, and home care and hospice. The outpatient part of this practice is located in a stand-alone clinic. This allows the provider to work independently yet have the specialist support and financial security from the entire Aspirus health care system. The critical access hospital has an average daily census of 9-10 patients.
The hospital and clinics provide numerous services and has up to date technology, including 64 slice CT and a mobile MRI. The medical staff includes family practitioners, internal medicine, general surgeons, orthopedic surgeons, radiologist, emergency medicine physicians, Nurse Practitioners, and Physician Assistants. Visiting medical staff includes ophthalmologist, oncologist, cardiologist, ENT, podiatrist, and nephrologist.
