The RN Case Manager will support clients and caregivers throughout our Indianapolis-West (Avon) territory.
A typical day for a RN Case Manager includes:Conducting initial evaluation visits and regularly re-evaluating clients' clinical needsCounseling clients and their families regarding clinical and related needsCoordinating and creating service plans for clients following client assessments and admissionsAssisting and supervising field staff, ensuring successful implementation of new Medicaid Waiver programsEnsuring quality care is delivered to our clients by creating, implementing, monitoring, and following quality assurance policies and proceduresAssisting with pre-service training and orientation, as well as in-service training for caregivers
Qualifications:Current State of Indiana or multistate RN licenseOne (1) year of RN experienceCurrent CPR certificationValid driver's licenseAccess to to reliable and insured transportationProfessional written and verbal communication skillsHome care experience is a plus, but not required
