Sholom
The Registered Nurse (RN), Hospice provides end-of-life care to patients with a terminal diagnosis needing hospice services in their home or community setting. This position will be cross trained into Medicare Home Care. There is an on-call rotation every fifth weekend.
PRIMARY RESPONSIBILITIES
Provide nursing services for the patient while maintaining his/her dignity and privacy, with a goal of enhancing quality of life.Collaborate and coordinate nursing services with other staff and departments in providing care to patients.Document services provided to meet regulatory requirements.Document patient problems, nursing assessments, patient goals, care provided, and patient/family outcomes from interventions and care provided.Keep statistical and other program measuring outcomes, including maintaining and submitting quarterly records of time spent, referrals and assessments, patient outcomes and satisfaction and related activities.Encourage timely consideration by Sholom staff for Hospice as an alternative mode of care, assisting in identification of appropriate patients for the purpose of improved palliation of symptoms and supportive care for terminally ill patients.
SECONDARY RESPONSIBILITIES
Perform miscellaneous projects and completes various tasks as requested by management.
JOB SPECIFICATIONS
Education, Experience and Credentials
• Graduate of an accredited school of nursing required.
• Must be licensed as a Registered Nurse in the State of Minnesota and must maintain licensure.
• One or more years’ recent nursing experience in home care, hospice or public health required.
• Current CPR certification and maintenance of certification required.
