Registered Nurse Case Manager (Part-Time)

East Bay Innovations (EBI) partners with Long Term Care Services and Supports providers in Alameda County. EBI prioritizes high-touch care coordination for individuals with chronic health conditions like chronic kidney disease (CKD), Congestive Heart Failure (CHF), and others. This is an interdisciplinary model where care is developed and managed by the client alongside the EBI care team and community providers. For patients with chronic health conditions, that means introducing health education, reducing hospitalizations, and focusing on preventative care treatment and healthcare access.

About the Position:

The Registered Nurse Case Manager will be one of the EBI care team members of this intensive, adaptive, and client-focused care model. EBI is looking for nursing professionals that understand the importance of high-touch interactions between client and healthcare provider to enhance client healthcare access, health education, improve health outcomes, reduce utilization, and improve the overall well-being of the client.

As an RN Case Manager, you will manage the medical needs of a caseload of patients as they navigate the staged progression of chronic kidney disease (CKD). RN Care Managers will manage acute and post-acute episodic interventions, including condition assessment, SNF coordination, education, etc. in the patient’s home and virtually. Collaborate with the Care Team and physicians to prevent readmission, prevent future hospitalization, and improve patient care.

Education / Experience:

Degree in Nursing is requiredTwo (2) years RN experience minimumActive Registered Nurse License - California Board of Registered Nursing (BRN)Experience working with chronically ill (CKD/ESRD) patientsHome care, case management, and/or hospital experience preferredRN’s with acute care experience within the past 5 years is preferred – ICU, CCU, ED, Stepdown, Nephrology, Renal Transplant or Dialysis RN’sBilingual in Spanish, and must pass a language proficiency exam.

Locations:

This role will require up to 80% weekly travel throughout Alameda County (mileage is reimbursed).Possession of valid California driver’s license with a driving record that meets agency standards, auto insurance, and access to a vehicle.

Schedule:

Part-Time: 16 hours per week during business hours (Monday-Friday).Schedule to be determined based on availability.

Compensation: 

$67.56 per hour.

Duties and Responsibilities:

Can build strong, productive relationships and maintain consistent communication with patients, their families, and providersCollaborate with Care Team and physicians to prevent readmission, prevent future hospitalization, and improve patient careProvide high intensity engagement with patients and family. Introduce self to patient/family and explain nurse case manager role and process to contact nurse case manager for questions, guidance, and educationEnhance a collaborative relationship to maximize the patient’s/family’s ability to make informed decisionsManage a caseload of nephrology practice patients and perform clinical needs assessments/complex-medical management for high-risk CKD/ESRD patientEducate patients using kidney disease-specific education developed to be culturally appropriate and tailored to a diverse set of learning needsAct as a patient’s advocate, liaison, and information resource; escalating all appropriate information to their nephrologist as appropriateAssess and assist patients for linkage and utilization of appropriate medical and community resources in conjunction with other members of the care coordination teamComplete timely and accurate documentation per company-documented procedures and relevant professional and regulatory standards
