Under the general guidance of the Director of Patient Services (DPS), the Home Health RN Case Manager performs patient care and care coordination duties for the patients of the Agency; as well as provides supervision for home health care registered and licensed practical nurses and home health aides. The RN works within the scope of practice as defined by the NYS Education Department for registered professional nurses. The RN Case Manager practices his\/her duties in a manner that is compliant with federal, state, and local codes, rules, regulations, and law. The RN Case Manager will practice competent clinical skills and perform timely accurate documentation as evidenced by clinical records audits and supervisory visits and chart reviews, and will educate patients and families in areas of knowledge deficit. \n

Roles and Responsibilities:\n\nResponsible for the initial assessment, plan of care development and revision for nursing and aide services, referral to ancillary services (PT, OT, SLP, MSW, HHA, and community services), ensuring appropriate physician orders are obtained, initiate case conferencing with other disciplines. \nSupervises home health care registered and licensed practical nurses and home health aides.\nEnsures quality and safe delivery of home health care services.\nImplements current nursing practice.\nGives accurate information to patients, patients' families, and home care staff members regarding costs for home health care services.\n\n

Required Skills and Experience:\n\n\n
The successful incumbent must possess a Registered Nurse Certification from an accredited nursing program and have current licensure in New York State. Preference will be given to candidates possessing Bachelor's degrees and high tech skills, as well as those who have experience with the Agency's medical data base systems. The following is also required:\n\nA minimum of three to five year's nursing experience with at least two years in home health care or public health nursing.\n\n\nCompetent clinical skills at a level that allows for independent, accurate, and thorough patient assessment and provision of required nursing procedures in the home setting.\nCertification in a specialty area or possession of 3-years' of experience in a specialty area (must be documented). Examples of specialty: Wound Care, Pediatrics, PICC lines etc. \n\n\nOrganizationally skilled, detail-oriented and able to problem-solve.\nMust have a valid NYS drivers' license, pass a motor vehicle and a criminal background check. \nMust provide immunization record and physician's statement of health at hire and upon return to work from illness-related absence of more than 3 days. \nA high degree of confidentiality is required. \n\n\n\n

Pay Range Disclosure:\n\n$75,000 to $80,000 per year\nTravel mileage reimbursed.\n\n
