Registered Nurse - Heart Failure Program Coordinator Location: The Good Samaritan Hospital, Lebanon, PA Schedule: Full Time Full-time 80 hours bi-weekly Must have a minimum of 3 years acute care experience Must have an understanding of CHF Must have leadership skills General Summary Develops, implements improvements, and manages the Heart Failure Program at assigned entity. Duties and Responsibilities + Introduces and recruits patients to the Heart Failure (HF) Program. Services as a liaison between the patients, families and healthcare team during hospitalization and the post-discharge period. Communicates to Cardiologist and hospitalists and other hospital staff regarding abnormal findings in evaluation of patients and assesses readiness for discharge and risk for readmission. Provides bedside education to patient and care giver(s). Administers follow up phone calls to all hospitalized HF patients and addresses all problems that require attention to improve patient perception of care. + Assumes responsibility for the development, implementation, and management of the disease specific certification for HF. Develops, implements, and maintains, and updates all required documentation for Joint Commission/DNV disease certification including required agreements with ancillary services, logbooks, licensure/board certifications, performance improvement activities, written care protocols, continuing education credits for physicians and nurses on staff and organizational charts. Ensures that the System adheres to the disease specific standards and is prepared for site visits. + Participates in the development, maintenance, implementation, and evaluation of written care protocols based on established guidelines from the American College of Cardiology (ACC), American Heart Association (AHA) Get with the Guidelines and the Heart Failure Society of America (HFSA), including physician orders, clinical pathways for expected length of stay and clinical practice guidelines. Routinely communicates with other program coordinators and Director, Heart Failure to adopt best practices and share ideas across the system. + Ensures data is collected, reviewed, and analyzed following the American Heart Association Get with the Guidelines protocol. Develops and implements action plans accordingly based on outcomes data. + Develops and implements the patient education process for all hospitalized Heart Failure patients with emphasis on Heart Failure symptom recognition, risk factor modification and structured Care Coordination. Revises and monitors educational materials to ensure that they are coordinated, understandable and meet the program standards. + Serves on/leads a variety of Heart Failure related committees, including but not limited to HF Clinical Effectiveness Team (CET), entity specific HF Steering Committee, and Hospital Quality Committees. Provides entity specific updates to the Heart Failure (CET). Chairs HF Steering Committee. Prepares agenda and progress reports for the team that discusses causes of readmission in patient care and identifies outcome measures that have not been met. Contributes to total cost of care reduction initiatives for HF population and leads initiatives put forth by the CET at the local level. Develops, implements, and evaluates action plans for improvement with the team. + Develops, coordinates, and participates in community awareness programs about HF disease management annually, and other outside groups to enhance patient care beyond the acute care facility. Common Expectations: + Serves as an active member of hospital and community committees that are related to Heart Failure patient care and community awareness programs. Represents the Heart Failure program at the local, state, and national level. + Assists with the development of the annual Heart Failure budget for operating and capital expenses. Ensures quality Heart Failure care is given while maximizing financial reimbursement and developing opportunities for cost savings. + Demonstrates motivation for learning through independent reading, professional networking and attending relevant conferences and seminars. + Provides outstanding service to all customers, fosters teamwork, and practices fiscal responsibility through improvement and innovation. Qualifications Minimum Education: + Bachelors Degree In Nursing (BSN). Required Work Experience: + 3 years Experience in nursing with 2-3 years of inpatient acute care. Required and + Medical/Surgical experience. Required + Heart and Vascular experience. Preferred and + CVS and Heart Failure care, care coordination, program management, performance improvement skills, leading meetings/committees, data analysis, and use of lean tools.
