**Position Highlights**
**Summary**
+ The Home Health and Hospice RN is responsible for client care to include direct nursing care, coordination of care, and coordination of services in the home for clients of all ages, unless otherwise specified, in order to assist the client in achieving a maximum level of wellness and independence.
**General Hours of Work**
+ Rotating shifts with weekday/evening & weekend/holiday call involvement
**Education and/or Experience**
+ Diploma, Associates or Bachelor's degree from a college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. Bi-lingual experience preferred but not required.
**Certificate, Licenses, Registrations**
+ Current Registered Nurse license in the State of Iowa. Must obtain CPR certification within 3 months of hire date. Access to automobile required. This position requires a valid driver’s license and must carry their own automobile liability insurance.
**Essential Functions**
+ Responsible for coordinating client services, case management, providing assigned clients with direct skilled care, and teaching patient care skills to the client and/or caregiver. Initiates preventative and rehabilitative nursing procedures individualized to the client and according to age specific needs. All duties will be performed within the scope of the Nurse Practice Act.
+ Develops and revises a plan of care based on the initial assessment, continual reassessment of client/family needs, client history, nursing diagnosis, physician orders, expected outcomes, and client goals to include discharge plans when appropriate. Utilize appropriate agency and community resources, be a client/family advocate and follow visit utilization guidelines and protocols. Formulates and implements appropriate teaching plans, communicates significant clinical findings and changes in status to the physician and other involved personnel, including the clients.
+ Thoroughly document all skilled and medically necessary care thoroughly, timely and accurately. The plans of care, the documentation and the care provided, must comply with Medicare, State, Federal, insurance and payer requirements as well as local agency policy. Maintains client and organizational confidentiality at all times. The RN actively participates in the QAPI (Quality Assurance and Performance Improvement) program and completes all required in-service programs to ensure standards are met and skills are kept current.
+ Conducts Home Health Aide supervisory visits, prepares and updates Home Health Aide’s written instructions and documents findings. The Home Health RN must ensure that clients receive appropriate care and all duties and activities delegated to aides are within the scope of the Nurse Practice Act.
+ Collects patient health data. Documents findings utilizing Meditech Home Care and completes appropriate OASIS time point completely and accurately. Identifies expected outcomes/goals individualized to the patient and develops a plan of care that prescribes interventions to attain expected outcomes.
+ Collaborates with the patient, significant others and health care providers in providing patient care. Attends and prepares for active participation in every other week care conferences. Contributes to the professional development of peers, colleagues and others. Decisions and actions on behalf of clients are determined to be in an ethical manner.
+ Considers factors related to safety, effectiveness and cost in planning and delivering patient care.
+ Assists with the intake of referrals in Home care assuring Medicare admission criteria are met and will obtain benefits and preauthorization of services for commercial payers as indicated.
+ Performs other duties and responsibilities as assigned.
