The Ambulatory Heart Institute Clinic: Enhanced Heart Care is a core element of the Smidt Heart Institute and focuses on the Heart Failure population. The Enhanced Heart Clinic works with patients during their Heart Failure admission and sees them post discharge, to help prevent or reduce any further Heart Failure admissions. You will work with Heart Failure inpatients, establish relationships with them, help provide Heart Failure education, and see them in the post discharge clinic, along with the Advance Practice Practitioners and pharmacist.
As a Registered Nurse in the Ambulatory Heart Institute Clinic: Enhanced Heart Care, you will assume responsibility and accountability for the application of the nursing process and the delivery of patient care within the Heart Failure patient population. In this role, you have the unique opportunity to establish relationships with this patient population during their inpatient stay, help with their care coordination, provide Heart Failure education, schedule a post discharge clinic visit and see them in the post discharge clinic. You will work in collaboration with the Clinic’s Advanced Practice Practitioners and a Pharmacist.
Perform patient assessments, triage patient concerns, draw clinic labs, deliver clinic mediations and provide clinical education to patients for Heart Failure and other disease processes. Perform inpatient rounds on Heart Failure patients. Triage phone calls from patients, pharmacies and other providersMeets with patients and families to offer transportation options and coordinates the trip.Makes post-discharge appointment in collaboration with the patient/family performs follow-up on post-discharge matters as requiredEvaluates patient needs/requests; reports observation and brings urgent and/or crisis situations to the attention of the team immediatelyDemonstrates a culturally sensitive approach to patient and familiesQualifications
Educational Requirements:
Graduate of an accredited nursing program. BSN or higher preferred.
License/Certification/Registration Requirements:
Current California State RN License required.
BLS Current
For RN III: Specialty Certification (example: CV Nursing) (external applicants may obtain within one year of hire)
Experience:
Minimum 3 years recent ambulatory or inpatient RN experience in specialty- Heart Failure. Note: (one (1) year experience will be considered for Registered Nurse II)
Working knowledge of applicable Standards of Practice.
Demonstrated dedication to customer service and ability to meet the needs and expectations of patients and health care colleagues.
