We believe every day is an opportunity to make care more personal and life more comfortable. That's why we offer personal care, palliative care and hospice to give people the care they need with dignity, grace, and love. As a Hospice **RN** Case Manager (Inpatient Unit) you will:
+ Report directly to the Executive Director – RN (Hospice Inpatient Unit) and will provide total patient nursing care in the inpatient unit facility.
+ Provide direct nursing care to patients.
+ Complete admissions, discharges, and complete nursing note documentation.
+ Prepare, administer, and chart medications and treatments.
+ Make rounds on all patients at the beginning of your shift and then periodically during each shift.
+ Accompany physicians on rounds at the inpatient unit.
+ Make assignments and supervise care and services provided by Hospice Aides.
+ Act as primary liaison between physician, patient/family and hospice team.
+ Provide direct patient care services to hospice patients and supportive care to the patient's family with a reasonable case load while providing a trusting and compassionate relationship with your patients and their families.
+ Empower patient's caregivers by educating them on care for their loved one.
+ Manage and collaborate with an interdisciplinary group consisting of Certified Nursing Assistants (CNA) and other support staff to provide comfort, peace, and dignity.
+ Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
+ Maintain all assigned patient clinical records.
+ Participate in company quality assessment and performance improvement programs.
+ May assist with home-based patients as needed.
**About You**
+ Bachelors/associate degree in nursing or equivalent
+ Registered Nurse license in the state of work.
+ Minimum of 6 months Registered Nurse experience
+ BHospice heart: compassionate, kind and empathetic to your patients and their families.
+ Comfortable providing care wherever the patient calls "home".
+ Demonstrate flexibility and composure while supporting others in a time of crisis.
+ RN license in the state of requested employment (or eligibility to obtain).
+ 1+ years of nursing experience in critical care or acute care (such as ICU, IPU, ER or emergency room) strongly preferred.
+ Additional, preferred experience to help you be successful: hospice, home health, oncology, hospital staff RN, Med Surg, PACU, Float pool, cardiac, geriatric, admissions, wound care, telemetry or travel nursing.
+ Strong critical thinking and crisis management skills.
+ Basic computer skills.
+ Current driver's license and automobile insurance.
+ Current CPR certification.
