Registered Nurse Mental Health Intake-Day/Pm- LOH Nursing Admin

Position Highlights:

Position: Intake RNLocation: LOH Naperville ILPart Time: 20 hrs/wkHours: 7a-7p; 3p-11p

A Brief Overview:
Provides a professional nursing assessment, prioritizing treatment according to the urgency of need. Completes level of care assessments with patients and/or family members and initiates pre-certification process. Processes direct and transfer requests from other facilities, reviewing both their medical and behavioral health assessments, reviewing appropriateness of transfer with Physicians and Administrative staff. Serves as link to the inpatient unit, completing nursing admission process with the patient. Serves actively and constructively as a member of the multi-disciplinary treatment team, supporting the organization and program philosophy of care to ensure the safety and well-being of each patient.

What you will do:

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs.Conducts medical screening of patients to ascertain if any medical issues need to be addressed prior to admission or if patient is medically stable to be admitted to Linden Oaks. Gathers all necessary information from patient, family and referral sources to make an informed decision. Collaborates with other staff when indicated.Assesses the clinical needs of patients seeking behavioral health services and determines the appropriate level of care that is consistent with the patient’s mental and psychiatric status and symptoms.

• Identifies potential/actual problems that may adversely affect the patient and/or department. Reports situations immediately.
• Oversees follow-up with referral sources as appropriatePartners collaboratively with the Assessment and Referral staff to ensure timely completion of patient transfers.Processes transfer requests from call to completion reviewing for appropriateness of transfer to LOH, consulting with the on call Psychiatrist and coordinating patient admission to LOH as appropriate.Conducts full admission process with patients and family members prior to unit RN assuming care to include but not limit to, body checks, MD orders, nursing care plans and education.Serves as a role model and change agent for corporate and department-based initiatives by demonstrating problem-solving, conflict management and customer service skills.Enhances interdepartmental relations by establishing good rapport and working relationships to foster intra- and inter-departmental communications necessary for the delivery of quality patient care.Demonstrates comprehensive understanding of hospital admission policies including, but not limited to, the Emergency Medical Treatment and Active Labor Act (EMTALA), Mental Health Code, Mental Health Confidentiality Act as well as regulatory agency standards (i.e. JCAHO, IDPH, and CMS).Demonstrates understanding of behavioral health reimbursement systems including managed care contracts, utilization management process, Medicare and Medicaid.Works with charge staff to optimize work flow in the department. Recognizes and responds appropriately to priorities in assignments; performs routine assignments independently, seeking direction when indicated for unusual or non-routine duties. Advises appropriate personnel of situations requiring additional nursing expertise or clinical follow-up.Collaborates and consults with Nursing Supervisor and Clinical Leaders and Managers to identify nursing clinical care needs, patient placement needs and concerns of patients.Maintains a clean and safe work environment, implements emergency procedures, drills and code procedures as necessary. Helps ensure the proper work/repair orders are dispatched.

2 Years of nursing experience1 Year of behavioral health experienceExcellent organizational and time management skillsBSN Degree or actively pursuing BSN DegreeCurrent State of Illinois Registered Nurse licensure Required AndCurrent CPR Certification issued by the American Heart Association or Red Cross Required AndCPI (Crisis Prevention Intervention) certification within 90 days of start date in position Required And
