As a Nurse Navigator for Pediatrics, a member of the core leadership team, you coordinate care for patients throughout the continuum of treatment. From initial referral to follow up care, the Nurse Navigator serves as the consistent point person for the high risk patient.
You will establish and revise the plan of care for appropriateness and coordinate support services within the health system and community to meet patient and family needs.You will demonstrate clinical competence in the assessment and intervention for the patient, family, or significant other and consult with physicians and other health care providers as indicated regarding patient care and quality of care issues.You will provide leadership in developing standards of care for patients undergoing new therapies following the approved protocol for specialized nursing careYou will represent Pediatrics in CME programs, symposia and Cedars-Sinai community programs and/or perinatal health as well as at regional and national meetings and conferences.You will also be responsible for the growth of the programs associated with areas of responsibility and assurance of excellent patient and family centered care as well as continuity of care.QualificationsEducational Requirements:Graduate of an accredited nursing program. BSN required, MSN preferred.License/Certification/Registration Requirements:Current California State RN License required.American Heart Association Basic Life Support certification required. (Neonatal Resuscitation Program, Advanced Cardiac Life Support and Pediatric Advanced Life Support certifications must be obtained within 6 months of hire).Specialty Certification required (external applicants must obtain within one year of hire).Experience:Minimum 3 years recent acute RN experience.Perinatal, Neonatal or Pediatrics experience preferred.Experience with program development and implementation of navigator programs preferred.Experience in a leadership role preferred.
