Registered Nurse Navigator (RN) - Physicians' Office - Spine and Neurosurgery

RN Navigator – Spine and Neurosurgery  

Responsible for supervising and overseeing the care transformation initiatives of the Spine and Neurosurgery service line. Responsible for identifying, developing, planning, organizing and implementing current and future transformation strategies to optimize the quality, efficiency and cost containment of care delivered throughout the continuum. 

Required Qualifications:

           

Education:

Bachelor’s degree required

Master’s degree preferred

Experience:

Minimum of 3 years’ clinical experience

Background in spinal services, quality management, public health or care management

Licensure, Registration or Certification:

Current RN licensure with South Carolina State Board

Background in LEAN or Six Sigma practices preferred

Position Responsibilities:

Care coordination and communication amongst providers and clinical staff of comprehensive spine program, including, Ortho, Neuro Sx, Pain Mgmt, and Therapies.

Schedule patients and address clinical questions, ensuring they are matched with the appropriate provider (surgical or non-surgical based on history and protocols).

Provide patients with a direct contact on their after-visit summary for any clinical questions related to procedures or MRIs, referring them to specific providers as needed.

Manage spine and neuro-surgery ED referrals and route them to the correct providers.

Track all patients via an ongoing Epic work queue and regular follow up to ensure appropriate continuity of care.

Manage procedure and MRI follow-ups, contacting patients to schedule follow-ups with respective providers

Schedule patients with their established spine providers if they had prior care within BSMH.

Managed work queues and contacted patients within 24 hours of receiving a referral.

Collaborate with department staff to complete tasks, update records, document, chart, and schedule follow-up with patients
