Registered Nurse Optimization - Navigator

POSITION OVERVIEW:

The Bone Health Navigator RN plays a crucial role in managing and coordinating care for patients with or at risk for bone health issues, such as osteoporosis and fractures. This position involves comprehensive patient education, meticulous care coordination, and active collaboration with healthcare providers to ensure personalized and effective care plans. The RN will be a key resource for medication management, ensuring that patients adhere to prescribed treatments and understand their importance. This role also includes monitoring patient outcomes. The Bone Health Navigator works closely with healthcare providers to ensure comprehensive care and are empowered to make informed decisions about their bone health.

EDUCATION:

Graduate nursing program Follows most current standards for continuing education requirements as dictated by State/Federal licensure requirements. 

EXPERIENCE:

Minimum of 2-3 years of experience in a healthcare setting, with a focus on bone health or chronic disease management, preferred. Experience in patient navigation or care coordination is advantageous. 

LICENSE/CERTIFICATIONS:

RN MT licensure Basic Life Support

KNOWLEDGE/SKILLS/ABILITIES:

Medical Terminology Basic Nursing Principles and Procedures Strong understanding of bone health conditions and treatments Keyboarding skills.  Basic office skills.
