Responsibilities to include: • Coordinate and facilitate quality management for a diverse outpatient population across the continuum of care. • Provide skilled triage and assessment, patient education, and collaborate with healthcare and community resources. • Support the care team in the provision of comprehensive patient care. • Offer expert patient advice and guidance. • Assist in prioritizing daily clinic workflow. • Provide direction and oversight to team members, including Medical Assistants, ensuring clinical competency.
