Position Summary
The Home Health Registered Nurse Preceptor - PPV is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Health Registered Nurse Preceptor functions as an educator, mentor, facilitator, change agent, consultant, leader, and respected expert resource to new hires and all team members. S/he is responsible for orientation, education, development, and retention of newly hired team members.
Position Specific Responsibilities
Attends and participates in staff meetings, educational programs, and community events, as requested.Communicates identified needs and potential solutions to supervisor.Stays informed of current Medicare/Medicaid regulations.Evaluates to ensure the effectiveness of educational content and delivery as evidenced by charts maintained to meet Medicare/Medicaid regulations.Assists in the audit of records as requested to assess for compliance and understanding of education and educational content provided.Implements educational content to support the quality of care and services provided by the agency.Implements educational content while precepting and mentoring, and supports successful onboarding of new employees which improves engagement and decreases staff turnover.Assesses documentation to ensure it reflects the quality of care and services provided and identifies education required to improve documentation and quality of care provided.Collaborates between agency and team members to provide education and updates education to support effective onboarding, optimal outcomes, patient satisfaction, orientee satisfaction with onboarding, quality care, and compliance.Manages delivery of educational content throughout the agency to ensure successful onboarding of new team members/leaders, to meet ongoing home health care needs, and to ensure plans of care are updated to accurately reflect new orders, interventions, and instructions.Collaborates with Home Health Director, RVP, Director of Education, and Clinical Educator throughout orientee onboarding to promote optimal quality and outcomes, to support compliant QAPI program and any Plans of Correction following internal/external surveys/audits.Reviews staff turnover data and collaborates with the Clinical Excellence Department to determine the success and effectiveness of onboarding education as evidenced by reduction in staff turnover, to develop process improvement measures to improve this outcome.Completes routine and emergency assessments of patients as indicated by departmental policy.Provides coordination and education of interdisciplinary clinical care among all disciplines (RN Case Manager, PT, OT, ST, MSW, PPV RN, LPN/LVN, and HHA) as indicated by Plan of Care through case management.Maintains reduced patient caseload to facilitate precepting new hires for short periods, and when not precepting will be available to support patient care needs of program location.Supervises the care given by Licensed Practical Nurses and Home Health Aides.Records observations, treatments, and other pertinent information.Educates on and provides clear, concise communication to the interdisciplinary team (IDT) and attending Physician regarding patient/family needs and measures to alleviate symptoms, and monitors response to measures implemented.Educates on and provides physical and emotional intervention to support the patient and family in the period of crisis through case management.Educates on and prevents unwarranted hospitalizations by meeting patient and family needs in the home.Educates on and develops patient-specific plans of care and coordinates patient care, changes in condition, medication, or treatment in conjunction with the ordering Physician or NPP.Provides education on and collaborates with IDT to meet the needs of patients, changes in condition, medication, and treatment.Educates on and collaborates with the IDT to coordinate home health care for the patient and family to ensure skilled need, continuity, quality of care, and concerns or updated orders.Provides education to orientees, patients, and caregivers regarding care of patients, disease process, symptom control, treatments, and procedures including, but not limited to, wound care and IV therapy.Educates orientees, patients, and caregivers on home health services and Medicare benefits, homebound criteria, and skilled needs, and admits patients to home health.Educates on and completes all OASIS assessments as required.Educates on expectations and documents care at bedside or in the patient's home as is possible.Educates on expectation and completes and submits clinical notes and OASIS, time logs, and all required documentation by 10 A.M. the following morning.Provides real-time feedback to team member orientee and supervisor throughout onboarding.Assists Administrator and Clinical Supervisor on process improvement plans for staff education and retention.Participates in on-call rotation as requested.Regular attendance is an essential function.Performs other duties as assigned.
Education and/or Experience
Associate or Bachelor's degree in Nursing required.Registered Nurse with a minimum of two (2) years of nursing practice in acute, critical care, or hospital care required. Experience in a home health or hospice setting preferred.OASIS experience required.
Skills
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.
Certifications, Licenses, and Registrations
Active and unencumbered license as a Registered Nurse in state(s) of employment required.Current CPR certification required.
Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
