Job Overview:
Organize and administers individualized age specific professional care to patients within an assigned unit of the hospital in support of medical care. Supports philosophy, objectives and the mission and vision of Saint Barnabas Medical Center and the Department of Nursing. Practices in accordance with the State Nurse Practice Act, American Nurses Association Standards of Practice, Code of Ethics for Nurses, Department of Health Regulations, TJC requirements and other regulatory agencies. Participates in patient teaching and discharge planning. Participates in unit Performance Improvement evaluation activities.

Qualifications:

Required:

Must be a graduate from an accredited school of nursing Excellent communication and interpersonal skillsOrganizational ability and leadership skillsDemonstrated clinical expertise in area standardsCompletion of all orientation requirements of CBMCSuccessful completion of all orientation programs

Preferred:

Baccalaureate Degree in Nursing or currently matriculating

Certifications and Licenses Required:

Current Registered Nurse License in the state of New JerseyBasic Life Support (BLS) from the American Heart AssociationPALS certification from the American Heart Association within 6 months of hireMust be I.V. certified from an approved facility or completion of requirements for I.V. certification within 90 days of hire to unit

Scheduling Requirements:

Essential Functions:
Assists patient in the use of ambulation techniques and assistive devices such as: commode chair, walker, hip chair, crutches, etc. and positioning of patient. Assumes Charge Nurse functions as delegated by the Nurse Manager, including assessing staffing needs, organizing and delegating work assignments based on patient acuity and individual staff skill levels. Completes daily patient classification according to facility policy. Complies with Medication Reconciliation policy. Complies with unit and departmental scheduling guidelines. Consistently conducts patient rounds as per unit/department schedule and addresses patients with standard questions. Delegates and prioritizes work assignments according to the individual associate's competence and patient care needs. Demonstrates proper procedure for Foley Cath use including assembly of required equipment, proper insertion and completion of chart notes. Demonstrates understanding of hospital policy and procedure regarding restraints. Demonstrates understanding of/states blood administrative hospital procedure. Evaluates effectiveness, interventions and revises plan of care accordingly. Implements the policies and procedures of the hospital and Nursing Department utilizing the nursing process with a multidisciplinary approach in the delivery of patient care. Initiates IV sticks, maintains IV sites and monitors IV fluids. Provides IV care and documents using established policies and procedures.

Initiates/develops multidisciplinary care plans and discharge plans in accordance with department standards and using appropriate tools, e.g., patient care pathways. Integrates psychosocial, physical, nutritional and rehabilitation needs of the patient in-patient care and discharge planning. Performs and documents an accurate/appropriate physical assessment as indicated by the patient's condition. Performs and documents accurate/appropriate physical assessments as indicated by the patient's condition. Performs assessment and reassessment of patients according to applicable department standards and identified tools. Performs interventions in accordance with current policy and procedure, safety guidelines and infection control principles. Prepares the environment to receive the post-operative patient from PACU in a timely and efficient manner. Provides appropriate pre and post-op care and education to patients and families as per established standards of care, policies and procedures. Provides appropriate wound management prevention and care and documents based upon established policies and procedures.

Provides patient/family education according to the plan of care and documents same. Recognizes signs and symptoms of complication of Pain Management Therapies and intervenes appropriately by applying the principles of acute and chronic pain management to assess each patient s individual needs and provide appropriate and specific treatment and care: a)PCA b)PCEA c) Epidural d) block. Sets goals and formulates appropriate multidisciplinary interventions to meet these goals. States proper mechanism for activating cardiac respiratory arrest. Demonstrates understanding of code cart check and procedure for replacing items on code cart. Transcribes MD orders accurately and administers medication in a safe and accurate manner, demonstrating knowledge of action and potential side effects. Transmits pertinent patient information to appropriate individual (i.e., MD, supervisor, discharge planner, etc.) uses SBAR for handoff communication. Uses an understanding of the principles of growth and development to assess each patient s age-specific needs and provide age-specific treatment and care.
