Registered Nurse (RN) Job Details Level Experienced Job Location GEN Putnam - Putnam, CT Position Type Full-time Education Level 2 Year Degree Job Shift Day Job Category Nurse Description Registered Nurse (Full-time - 40 hours/week) Location: Putnam, CT (Behavioral Health Department) Generations Family Health Center, Inc. is a regional community health center covering a thirty seven town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam. Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral health care, as well as care coordination and community outreach. In addition to our main facilities, we offer mobile programs and School Based Health Centers. Registered Nurse Role: + Assess and identify patient needs to ensure that safety and high-quality care is provided through nursing interventions, supervision of staff, education, patient advocacy and coordination of services. + Develop and maintain respectful and cooperative working relationships with other professionals, to contribute to the integrated, seamless delivery of services to individuals and families in the communities we serve. + Works in collaboration with an interdisciplinary clinical team to provide specialized behavioral health support to individuals who are experiencing mental health and/or substance use challenges.
