When illness develops and is significant enough to require a hospital stay, Hospital From Home provides a similar care delivery model for patients as an alternative to a standard hospital stay. This model brings all the essential elements of inpatient care to the comfort and convenience of a patient’s home. Hospital From Home offers a more advanced concept than traditional home health care as the program provides at least 2 clinician visits each day to treat the patient’s condition(s) until they are safe to be cared for by their regular clinical team. Common conditions treated by Hospital From Home include Congestive Heart Failure, COPD, Pneumonia, skin infection such as Cellulitis, and Urinary Tract Infections as well as many other conditions deemed safe enough to treat in the home per the Provider. In this role the Hospital From Home RN supports care provided in a 60 minute radius surrounding Blessing Hospital working closely with the Site Coordinator and Provider. JOB QUALIFICATIONS Education/Training/Experience: REQUIRED: + Graduate of an RN School of Nursing + Excellent written and verbal communication skills + BSN + 2-4 years of inpatient experience as a registered nurse, within the last 5 years + EXCELLENT COMPUTER SKILLS + Ability to work various shifts/weekend availability + PREFERRED: + Experience providing RN services in the home, community , or clinic + Experience starting and managing peripheral IVs, midlines, and PICCs + Experience in phlebotomy License/Certifications/Special Requirements: + Possesses current State RN license. + Must be willing to apply for Missouri RN licensure within one month of hire. + Must have an automobile + Current driver’s license + Proof of auto insurance equivalent to state law requirement + Geographic coverage: Service area includes a 60 min. radius of Blessing hospital.
