The Cardiac Nurse Navigator will be responsible for the following:

Bridge inpatient to outpatient: ensure proper inpatient HF education, HF Clinic education and the importance of attending, and notification of the outpatient APP of upcoming appointments (in basket message). Ensure HF Care path metrics are met Review the readmission risk score. Identify high-risk patients and evaluate for resources. Partner with the case management team. Schedule patients in transitional care clinic.

Exceptional Skills and Qualifications

-3 years of inpatient nursing experience required; 5 years of experience preferred. Recent Cardiac experience is required.

-Preferred experience in case/care management role; home health or other independent role requiring nursing assessment, critical thinking and application

-Epic experience is a serious plus

-A clinical background and genuine interest in or connection to the healthcare industry is a serious plus

-Completion of a nursing program recognized by the appropriate state nursing board; BSN preferred

-Registered Nurse in Indiana with active IN license

-Certified Case Manager (CCM) preferred
