MAJOR FUNCTION: 

The Inspira Medical Group Ambulatory Registered Nurse will operate in collaboration with primary care practices and the healthcare team.  Primary responsibilities will include identification of Medicare patients appropriate for Annual Wellness Visits and execution of guided processes to complete the visit; clinical assessment and development of comprehensive plans of care appropriate for patients with complex medical, emotional, and social needs; and referral to community and social service resources in support of patient’s holistic needs. 

QUALIFICATIONS: 

Education & Experience: 

You must be enrolled in a BSN or MSN program from an accredited school of professional nursing within six months of hire or rehire and graduate on or before the third employment anniversary date into a nursing position. *  BSN or MSN graduates preferred *Grandfathered Exemption of Educational Requirement:  Hired on or before May 31, 2013 -- IMC Hired on or before August 4, 2015 – IMCW Hired on or before May 31, 2023 – IMCM 2 to 5 years of direct patient care experience required. Experience in provider practice or clinic health setting preferred. Experience in a care management role preferred.  Experience in caring for patients with chronic disease required.

Certification/Licensure:    

Active New Jersey Registered Nurse License is required Active Basic Life Support (BLS) certification is required.

Knowledge & Skills: 

Familiarity with health IT systems and data reports. Familiarity with community and social service resources, navigating patients through the healthcare continuum, and working with disparate populations.

Physical Requirements:     

Occasional travel may be required to various primary care offices within Inspira Medical Group.  Work location will be a hybrid of home-based and office-based sites. Physical and mental capacity to perform responsibilities as outlined. Willingness to conduct video-based telehealth services to patients.
