The Palliative Care RN Case Manager is responsible for coordinating care for this clinic’s patients, including refilling medications, performing symptom assessments by phone or in person, working with inpatient and outpatient social work to provide patient services and equipment, and communication with hospice and home health agencies, SNFs, Adult Family Homes and patient family members regarding their care. **POSITION HIGHLIGHTS:** + Conveniently located near public transportation + Make a difference for the patients and employees we serve + Opportunity to provide career growth **PRIMARY JOB RESPONSIBILITIES:** + Nursing management of patients seen in the Palliative Care Clinic. + Manage some Palliative Care Service patients’ care independently (under the supervision and consultation of the Palliative Care Attending and Medical Director). + Perform patient teaching regarding palliative care as needed. + Triaging of phone calls from patients and caregivers and referring providers. + Assist team coordinators with scheduling (including the inpatient Palliative + Work with outpatient clinicians to carry out palliative care treatment plans for patients. + Processing of complex durable medical equipment referrals. + Participates in care conferences when appropriate. + Provides episodic and hospital follow-up for patients and families as needed, including weekly contact patients for pain and symptom assessment. + Ability to navigate an inpatient setting to facilitate outpatient palliative care referrals and provide a warm-handoff for admitted patients at Harborview. + Participates in weekly IDT Rounds with Inpatient/Outpatient for any hospitalized outpatients. + In conjunction with other members of the Palliative Care team, share responsibility for providing palliative care clinical education and support to physicians, nurses, social workers, chaplains, and others as requested. + Participate in development of patient and caregiver educational materials and other activities to support and promote high quality care for patients and families. + Support quality improvement projects in palliative and end of life care conducted by the Palliative Care Service. **REQUIREMENTS:** + Washington State or Multi-State RN license required by start date (active and unencumbered) + At least Two years professional nursing experience working with people at the end of life, experience in hospice or palliative care strongly preferred + Current BLS AHA Health Provider card + Obtain certification as a hospice and palliative care nurse within one year + Effective interpersonal skills, leadership skills, and clinical expertise in the care of patients with life limiting illness + Interest in providing evidence-based palliative care; knowledge of palliative and end of life concepts, best practices, and quality measures **ABOUT HARBORVIEW MEDICAL CENTER** As the region's only Level I Trauma and verified burn center, Harborview Medical Center is a comprehensive healthcare facility owned by King County and operated by UW Medicine.
