Primary Function/General Purpose of Position
The Team Lead, Care Management is responsible for assisting in assigning, coordinating, and directing care management activities. In addition to carrying a daily caseload, the Team Lead works within the care management team as a daily resource and mentor to their team and serves as a link between the Care Management leadership team, staff, and other interdisciplinary and cross continuum teams and departments.
Essential Job Functions
Carries a case load assignment in accordance to responsibilities and job functions of appropriate licensure (RN or Social Worker). Develops, coordinates and monitors strategies for obtaining, organizing and allocating human and material resources required for care transition planning and managing clinically and socially complex patients.
Collaborates with other departments and physicians to assure the best possible patient outcomes. Resolves department problems in a timely manner, provides guidance and coaching to staff for better outcomes. Facilitates education to physicians and hospital ancillary departments regarding opportunities that would result in better quality, reduced unnecessary utilization, and ensure smooth, safe, timely transitions.
Facilitates education and training of department staff as it relates to Care Management processes. Takes a lead role in ensuring interdisciplinary interaction/communication. Promotes a proactive environment to assess customer needs and address as indicated.
Serves as a resource and role model regarding department issues that involve planning, implementation, and evaluation of care management activities and standardized practices and processes such as Advance Care Planning, LOS/LOC management, readmission prevention including use of predictive readmission tool and application of appropriate interventions, denial prevention related to medical necessity through proactive progression of care and addressing barriers (including tracking of avoidable days or delays), daily patient and team rounding, and processes to ensure smooth, safe and seamless transitions. Assists staff in meeting professional and departmental standards of care, organizational policies and procedures utilizing feedback and coaching to enhance quality of care.
Communicates effectively, in a timely manner with all internal and external customers. Serves as a liaison between leadership and staff, and between physician, nursing and other care team members, patients, families, and caregivers.
Works with team to ensure compliance on regulations from state and federal agencies and collaborates with management in the employment performance appraisal process.
Understands data relative to patient population and utilizes information to improve quality and reduce cost. Meets established deadlines for reports.
Recognizes and promotes patient’s right to self-determination. Advocates for patients as well as for the ministry.

Licensing/Certification
BLS Basic Life Support – American Heart Association (required)
Certification and/or licensure in field of study as required by state and federal law (required)
Case Management Certification (ACM or CCM) (preferred)
Education
Bachelor of Science Nursing, Social Work, or other healthcare related field (required)
Master’s degree in healthcare related field (preferred)
Work Experience
2 years experience in clinical setting (required)
1-2 years of experience in an acute care clinical setting (preferred)
