Position Highlights:

Requirements:

Education: Bachelor’s degree of Nursing. Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE)Experience: At least 2 years’ experience working as a registered nurse in the case management fieldLicensure: Maintain a current, unrestricted RN license in WA or compact state.Current Basic Life Support Certification

Responsibilities:

Find the most appropriate medical resources and providing optimal care for the patient Handle case assignments, review case progress, and determine case closureHelp clients achieve wellness and autonomyInteract with clients and develop effective working relations with medical teamHelp clients with continuity of care for specialty services and home health needs
