Position Purpose:
The RN Case Manager at Mercy Care Atlanta is a vital role that blends direct patient care with leadership in a multidisciplinary team. You’ll work across our four Atlanta locations, coordinating care for patients with HIV and/or HCV. This role centers on retention and adherence to care for positive clients, ensuring they receive the highest quality of care, with a focus on self-management, health promotion, and harm reduction.
What You Will Do:
Coordinate patient care across Behavioral Health, Substance Use, Dental, SDOH, and Diabetes services.
Oversee programs for medications, including STOPGap and long-acting injectables.
Create care plans and offer guidance on managing co-occurring chronic conditions.
Facilitate treatment adherence through direct patient support and follow-up.
Manage a caseload of up to 90 patients, collaborating with ID physicians to achieve retention and viral load suppression goals.
Complete progress and quality reports, including for the Ryan White program, and engage in health fairs or community events.
Minimum Qualifications:
Bachelor of Science in Nursing required; MSN or MPH preferred.
Active RN licensure in Georgia.
3-5 years of experience with target populations; at least 1 year with HIV-related care preferred.
