Position: Home Health RNLocation: Felton, CAType: Full-Time
Salary: Starting at $65/hour, depending on experience, open to discussion!Benefits: Medical, Dental, Vision, PTO, 401K, and mileage reimbursement
Job Description:
- Assess patients and their families to determine home care needs.- Perform physical assessments and review histories of illness(es).- Initiate and update care plans as needed.- Collaborate with patients and families to develop individualized care plans.- Identify discharge planning needs and ensure they are implemented before discharge.- Provide counseling to patients and families to meet nursing and related health needs.
Requirements:
- Graduate of an accredited school of nursing.- At least one (1) year of recent nursing experience as an RN or LVN/LPN (per state regulations), preferably in public health, home care, or hospice.- Current RN licensure in California and a valid CPR/BLS certificate.- Strong observation, verbal and written communication, problem-solving, and basic math skills; nursing skills per competency checklist.- Must be a licensed driver with a reliable, insured automobile in accordance with state/organization requirements.
