Make a Difference
The Primary Care Navigator will work as part of a multi-disciplinary team lead by the PCP for timely identification of patients’ needs and ensure coordination of patient services. They will educate on disease process, goal setting and will monitor patients identified for wellness, transitions care, chronic care and high-risk care management programs. The Navigator will help facilitate member’s access to services throughout the healthcare continuum; transitioning members from one level of care to another with supporting with available resources for seamless transition.
Community Health Network MD Anderson Cancer Center is a partnership between Community Health Network and MD Anderson Cancer Center, one of the world’s largest and most respected cancer centers. The partnership formed in 2022 elevated a prior affiliation between Community and MD Anderson Cancer Network®, a program of MD Anderson. This new partnership represents a full clinical and operational integration of Community’s cancer services with MD Anderson across all five of Community’s sites of cancer care.

Your exceptional skills
Two years or more of Oncology nursing experience required. OCN certification preferred, but not required upon hiring. ***OCN or ONN-CG certification required within timeframe established upon hire.***
Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience. Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC). BSN preferred.
