Community Care Center is a large inner-city Skilled Nursing Facility. We have patients of all ages with multiple levels of care, including long term care and a large behavioral health (psych) population.
We have a new Dialysis program coming in 2025Along with our medical/skilled patients, the most common diagnoses of the rest of our population is schizophrenia, bi-polar and major depressive disorders.The Staff and patients are welcoming and say hello as you walk through the hallways.We are located near W 43 ST & State Street, just off 90/94 and a few blocks from the train station.Job DescriptionPRN (Per Required Need) / On-Call position
MINIMUM COMMITMENT:
As a condition of hire, you must commit to working a minimum of two (2) weekend shifts per month. Weekend are defined as Saturday and Sunday. The shifts can be split up within the month or you can work them together, on the same weekend.In addition, you must also be intermittently available to pick up other weekday shifts. You should have flexibility to work different shifts (Days, PMs and/or NOC). Our greatest needs are on the PMs and NOC shifts, but you may infrequently be offered to work a Day Shift.You must be available to attend a weekday New Employee Orientation.In addition:
This is an “as-needed”, intermittent/on-call schedule. If we have a need, you will be contacted to come in to work. Sometimes we know our scheduling needs in advance and sometimes you can be called in at the last minute.There is no guaranteed number of hours for this roleNo benefits or insurances are offered for PRN statusThis is a non-union positionQualificationsFloor Nurse: We will consider a Registered Nurse (RN) or Licensed Practical Nurse (LPN) for this role.
What we need from you:
