Responsible for managing and navigating Women's Service Line Outreach Education by providing program design, implementation, and evaluation of program initiatives to the designated service. Responsible for Perinatal High Risk and Bereavement care coordination. Guides patients and families through the healthcare system, from diagnosis to follow-up care, assisting with scheduling, transportation, referrals, and answering questions. Provides education and advocacy in line with service line policies. Acts as a liaison and advocate, helping patients access screening, treatment, research, and support services. Refers cases to appropriate resources and advocates for patient rights and service utilization. Assesses patients' health and psychosocial needs, developing a case management plan in collaboration with the patient and support system. Serves as a single point of contact for patients, helping resolve barriers to healthcare access, including financial and psychosocial issues, and facilitates continuity of care. Provides emotional support, counseling, clinical education, and guidance to patients and families, promoting informed decision-making and active participation in care planning. Promotes healthcare programs and builds relationships with community resources and providers to resolve barriers and improve patient outcomes. Manages complex cases, ensuring smooth transitions through the healthcare system and collaborating with multidisciplinary teams to address patient concerns. Communicates with payers to resolve reimbursement issues and ensure coverage for patients. Strives to improve patient satisfaction and outcomes by reducing emergency visits, readmissions, and transition times. Work schedule: This is a full time exempt role, hours are Monday-Friday | 40 hours per week Education: Bachelor of Science in Nursing (BSN) Experience: Minimum three years of Registered Nurse (RN) experience. Preferred Experience: 3 to 5 years of Registered Nurse (RN) experience in primary care management for patient populations, with strong women’s health knowledge Certification: BLS from American Heart Association upon start | Licensure: Licensed as a Registered Nurse (RN) in the Commonwealth of Virginia upon startRegistered Nurse (RN) Women’s Patient Care Navigator Preferred Qualifications
