The RN Coordinator does not work remotely, as they see patients in the Advanced Heart Failure Therapies and Transplant Clinic. The RN will monitor patients with their remote HF device such as CardioMEMs, Endotronix, or heart failure management system (HFMS).
Remote monitoring role of RN coordinator does all of the background work for the device- eligibility, prior authorizations, communication to reps about implant dates, and education prior to procedure date. Once implanted if there is monitoring involved the remote monitoring RN follows patients as “eyes in the sky” and communicates any changes to their cardiologist. Current devices are CardioMEMS, Barostim, and Zoll-HFMS.
Cardio-Oncology role of RN coordinator manages patients that are or at risk for cardiotoxic shock during active treatment and sometimes 1 year post-treatment.
Major Responsibilities:
Assesses organ failure patients during clinic visits, arranges for routine diagnostic testing and physician consultation. Communicates with clinic physicians, referring physicians, and other health professionals regarding patient condition and needs.Provides comprehensive teaching and counseling to patients and families. Identifies and addresses potential deterrents to compliance with medical regime.Monitors, evaluates, and communicates relevant data regarding clinical problems to appropriate members of the interdisciplinary team.Provides teaching and discharge planning in collaboration with hospital nursing staff.Monitors progress of hospitalized organ failure clinic patients and collaborates with other team members to ensure continuity of care.Follows-up on patient condition as needed, by maintaining telephone contact when patient returns home.Continues to provide continuity of care as patient is treated in an outpatient or clinic setting.Maintains organized and accessible medical records.Participates in ongoing medical and nursing research activities relative to organ failure patients.Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures.
