POSITION PORPOSE
Registered Nurse, Structural Heart Coordinator:
The coordinator will leverage their experience with cardiac, procedural, surgical services, cath lab or similar are responsible for assisting patients, families and providers through the continuum of service offerings to resolution of care. Ensures the availability, access and timeliness of services in order to optimize quality of care, coordination and patient experience.
Full-Time position scheduled 40 hours per week.
WHAT YOU WILL DO
Ensures the availability, access and timeliness of services in order to optimize quality of care, coordination and patient.
Coordination of information and patient transfer to other hospitals that have availability to do valve procedures.
Works with patients, providers and caregivers to identify possible barriers to care and systematically helps to break down barriers (i.e. Financial, transportation, language, etc.).
Facilitates as necessary, timeliness of appointments; works continuously to reduce the intervals between time of abnormal results, diagnosis and treatment.
Provides and/or facilitates clinical and emotional support, education and resources to patients and their families.
Monitors and tracks all patients who have been diagnosed with heart valve disease specifically but not limited to:
Maintain list of patients for aortic balloon Valvuloplasty, Mitral stenosis, pre TAVR patients, and patients with mitral clip.
Facilitate tests for patients; Prepare patient office records for presentation at valve clinic meeting (cath films, MRA, carotids, TEE, Lab Tests).
Post Procedure follow up.
MINIUMUM QUALIFICATIONS
Requires current Michigan Nursing License
Graduation from accredited nursing program, Bachelor’s degree required
Three or more years of progressive clinical nursing experience in cardiovascular health
